Payments, Insurance and Billing

Accepted insurances

We accept most major insurances including Aetna, Cigna, Blue Cross Blue Shield, Medicare/Medicare Replacement plans, Tricare Prime and Standard, United Healthcare, United Healthcare AARP, and some forms of Medicaid. However, there are some plans we do not accept, so please be sure to verify this information when you make an appointment.

Co-payments, co-insurance, and deductibles

If you have a responsibility for a portion of the payment associated with your plan, we’ll be happy to collect applicable copayments, coinsurances, and deductibles at the time of service. We currently accept payment in forms of Visa, Mastercard, Discover, American Express, Cash, and personal checks.

Insurance authorizations/referrals

If your insurance requires you to have a referral or authorization to be seen, please obtain prior to your appointment. Failure to do so could result in your appointment being cancelled.

CareCredit

CareCredit options

CareCredit is a simple and convenient way to consolidate and manage certain out of pocket healthcare expenses, because it is a credit card that can be used repeatedly and for any family member. CareCredit offers two basic payment options to meet an individual’s financial needs.

No interest

Interest will be charged to your account from the purchase date if the promotional purchase is not paid in full within the promotional period. Not all promotional plans are available at all Participating Professionals. Minimum Monthly Payments Required and may pay off purchase before end of promotional period. Each month you only need to pay the required minimum monthly payment and pay off the remaining balance within the promotional period to avoid interest and pay only the amount financed. Future minimum payments will vary based on amount and timing of payments, interest rate and other charges added to the account.

14.90% APR and fixed monthly payments

Not all promotional plans are available at all Participating Professionals. Fixed monthly payment amount based on repayment over 24, 36, 48 or 60-month period. Purchases of $1000 or more are eligible for a 24, 36 or 48 month offer and purchases of $2500 or more are eligible for a 60 month offer.
